Aleksey Kaigorodov put the hosts in front three and a half minutes into the period, but veteran Viktor Kozlov leveled the score for Salavat less than two minutes later.
The third period was scoreless and the teams headed to overtime, in which Metallurg achieved the upper hand.
Tomas Rolinek set up Pavel Zdunov for the winning effort with 2 minutes and 54 seconds played in the extra 20 minutes.
Game 4 of the tie is to take place at Metallurg Arena on Wednesday, March 30.
